Improving the financial management of State healthcare institutions

Subject The article considers the optimization of finance management of State healthcare institutions.
Objectives The aim is to clearly define the limit of social guarantees provided through public budgets, and create new mechanisms of social protection of citizens within the pursued public policy.
Results We have built a matrix model of efficient cooperation of medical establishment with insurance companies. The model serves as a method of assessing the effectiveness of investment projects. It operates by two major variables, i.e. a discount and anticipated increase in demand for services, and enables to determine the efficiency of project implementation by selected groups of services and the overall commercial activities of the hospital.
Conclusions and Relevance This model is universal and can be used to assess the performance of public and private medical organizations. In addition, the matrix model is suitable for embedding a scenario model into it, that is the assumption that the expected increase in demand may vary in three different scenarios: negative, optimistic and the most probable one, on which basis the weighted average expected increase in demand is determined.

Subject The article considers issues of improving the management of local funds of the compulsory medical insurance.
Objectives The aim of the study is to determine the current condition of territorial budgets of the compulsory medical insurance funds in the Volga Federal District, and to develop methods for their integrated assessment.
Methods The study employs methods of economic, systems analysis, and mathematical statistics.
Results We developed tools to assess the efficiency of local budgets execution of medical insurance funds in the Volga Federal District. The tools enable to create necessary conditions for effective operation of medical institutions operating in the market economy. The paper offers a methodology for integrated assessment of local budgets of the compulsory medical insurance funds on the basis of total standardized ratio subject to the created system of relative indices.
Conclusions The paper justifies that increasing the effectiveness of health management at all levels, and especially of territorial funds of compulsory medical insurance is an urgent economic and scientific-practical problem of the industry reforms.


Improvement of methodological tools to assess the health care industry using the resulting standardized index

Subject The article deals with the issue of adapting the public health service to the market economy conditions.
Objectives The paper aims to develop an integrated assessment methodology based on the resulting standardized index, taking into consideration the formed system of relative indicators.
Methods Using the methods of economic, system analyses and mathematical statistics, we researched the current state of the health care industry in three groups of countries: the Group of Seven, CIS countries, and BRICS countries.
Results We propose our own integrated assessment technique based on the resulting standardized index, taking into consideration the formed system of relative indicators that take into account a controversial and multidirectional character of countries. We considered the different relevance of evaluation system indicators using the method of expert ranking indicators (Fishburn's point-estimation rule). Given the performance data of the World Bank, we held a ball score of the health-care industry using the resulting standardized index.
Conclusions and Relevance The proposed methodology for the assessment of the resulting standardized health index in different States allows defining the country, implementing an effective strategy for the management of the health care industry. The methodology developed can be used not only to evaluate the resulting standardized health index worldwide but also to assess this indicator in a single country.


Applying the ABC and XYZ analysis by health care institutions: The public health facility Gorodets Central Raion Hospital of the Nizhny Novgorod Oblast case study

Subject Today, health care organizations should ensure economic performance and implement strategies of compensation for both standard and actual costs (at the expense of their budgets, compulsory medical insurance and income-generating activities). Along with traditional estimates of medical and social efficiency, any healthcare facility should apply a cost-benefit analysis of its operations.
Methods The paper employs methods of economic and systems analysis and mathematical statistics to study the current status of costs of a healthcare facility.
Results We offer a technique to analyze expenditure items, using the ABC and XYZ methods being the best tools to improve the efficiency of the health system. Based on the data of the public health facility Gorodets Central Raion Hospital of Nizhny Novgorod Oblast, we analyze the items of economic classification and characterize each group of expenditures for efficient management purposes.
Conclusions Using the combined ABC/XYZ analysis will improve the efficiency of cost management by public institutions. The management of the institutions will be able to keep track of key positions and reasons that have an impact on the amount and movement of costs. The technique will enable to properly filter costs by groups, set priorities, and make an accurate forecast of expenditures to plan financial and economic activities for the ensuing year.
